Alt text is a legal accessibility requirement in many contexts and a real SEO signal for image search. The distinction this tool makes, which many do not, is between an empty alt (correct for decorative images) and a missing alt attribute (always wrong).
alt="" tells a screen reader the image is decorative and to skip it, which is correct behaviour. A missing attribute leaves the reader announcing a filename or nothing at all. This tool counts them separately.
Long enough to convey what matters, usually under 125 characters. If the image needs a paragraph, put that paragraph on the page.
Only where they genuinely describe the image. Stuffing alt text is an old spam tactic and it degrades the experience for the people the attribute exists to serve.
They cannot have it. That is one reason meaningful images belong in an img tag rather than a CSS background.
